It shouldn't. If his large and small intestines are pressing up in front of his stomach and then flattening themselves back against his spine, he's probably in the final stages of disembowelment as the torturer goes for his nuts. Sheesh.

This is in the context of the guy being startled. I'm not even sure what he's trying to describe. Shitting in his pants? Clenching his stomach?
